Whilst the greatest effort has been made to ensure the quality of this text, due to the historical nature of this content, in some rare cases there may be minor issues with legibility. That the book may be useful, however, in preserv ing much valuable and curious information, and at the same time by giving a continuous - even if frequently interrupted - historical narrative in a single volume is my sincere hope.
